What to report?

All incidents affecting patient safety or potentially affecting patient safety

You should report an incident: • involved a clear error, even if it did not result in treatment correction / change of treatment • as above, but with a potential of resulting in an accident • required treatment correction • resulted in irradiation of radiotherapy professionals • where treatment corrections can not be facilitated, but where negative consequences for the patient are unlikely • where treatment corrections can not be facilitated, but where negative consequences for the patient are likely to occur
