Craniospinal irradiation case • A patient was treated with 39 Gy in 13 fractions to the craniospinal axis.

• A shift was foreseen after 7 fractions.

• As result of a mistake an overlap of 1 cm was created instead of a gap of 1 cm after 7 fractions. This mistake was applied for 2 fractions. • In the overlap region of 1 cm, the spinal cord received 7x 3.1 Gy Dmax and 2 x 6.2 Gy.
