Abstract:

The use of magnetic resonance (MR) guided radiotherapy (MRgRT) is of growing interests. One of the possible benefits is the opportunity of replacing computed tomography (CT) with synthetic CT (sCT) for dose calculation purposes. This, among other, could benefit the patient with reduced ionizing dose exposure, less hospital appointments, reduced MR-CT registration uncertainties and overall simplified workflow for the clinical personnel. Recent research demonstrated the viability of synthetic CT in accurately computing radiation dose. The findings suggest that synthetic CT demonstrates suitability for dose calculations, presenting a promising alternative to traditional CT simulations. While commercial solutions for synthetic CT are readily available for certain body sites, there is a noticeable absence of consensus and international guidelines regarding the performance of quality assurance in MR-guided radiotherapy using synthetic CT. This gap underscores the need to explore and present current methods available for quality assurance in the absence of established standards. This presentation aims to address this gap by presenting an overview of the existing methods for quality assurance in the context of MRgRT with synthetic CT. By reviewing and highlighting the current landscape of available approaches, we aim to contribute valuable insights to the ongoing discussions and efforts within the field. As an example, the table below summarizes multiple approaches that have been presented in literature to validate on a patient base (patient specific quality assurance) the dose calculation on sCT. Each of the proposed methods has valuable aspects and shortcomings. These points will be the starting point for the discussion during the pitch session.
