Results:
The median follow up was 8 months and all patients completed 6 month follow up.
The clinical and the metabolic response correlated in all the patients with 2 patients having both clinical and metabolic complete response by Day 20 & the remaining 3 patients by the end of EBRT. All patients recieved at least 5 cycles of concurrent chemotherapy.
All patients completed treatment within the scheduled treatment time with bare minimal toxicities.
Only Grade 0 or 1 Gastrointestinal/ Genitourinary and haematological toxicities were observed in all patients.
Conclusion:
IGOART -SIB is a feasible, well tolerated, patient friendly and efficient technique for pelvic malignancies, with reduced treatment related acute toxicities and increased patient compliance.
The On Couch Time can be optimised with experience and expertise.
PET CT based SIB facilitates dose escalation which can translate to an Increased BED, whilst ensuring no increase in toxicity, Which can translate into an increased local control.Online Adaptive Radiotherapy is a feasible, well tolerated, patient friendly and efficient technique for treatment of Cervical cancer, with reduced treatment related acute toxicities and increased patient compliance.
Online adaptive RT is futuristic and with AI training, the process is expected to be more accurate thus improving the treatment quality.
The combination of both these techniques facilitates monitored precise dose escalation with maximal normal tissue sparing.
Longer duration of follow up and large number study will be required to prove this proof of concept.
