Conclusion:

Taken together, overexpression of CSC marker expression influences the outcome after adjuvant radiotherapy in this intermediate risk group. These may serve not only as potential markers for patient stratification, but also, for selection of patients for treatment intensification, in addition to established clinical parameters.

Palliative treatment of HNSCC: Quad shot regimen with immunotherapy – a single center experience

Purpose/Objective:

Palliative treatment of advanced/recurrent or metastatic HNSCC, especially in frail patients, is challenging. Immunotherapy has shown modest response rates in metastatic disease. Strategies to harmonize palliative radiotherapy and palliative immuntherapy are needed. We report our single center experience with hypofractionated palliative radiotherapy using the Quad-shot regimen.

Material/Methods:

Patients undergoing palliative Quad shot radiotherapy for HNSCC between 03/2020 and 10/2023 were analyzed. Systemic treatment administered in close proximity to radiation is reported.

Results:

We analyzed 20 consecutive patients scheduled for Quad-shot radiotherapy. Median age was 76 years (range 57 97), 16 patients were male. One patient presented with ECOG 0, 10 patients with ECOG 1, 7 patients with ECOG 2, and 2 patients with ECOG 3. In total, 8 out of 20 patients (40%) presented in the primary setting and were not eligible for definitive treatment. Eleven patients (55%) presented with metastatic disease and one (5%) with recurrent disease after surgery. Radiotherapy was administered with 4 bidaily fractions of 3.5Gy in 5 patients and of 3.7Gy per fraction in 14 patients. All patients were treated with VMAT and IGRT technique. Of 20 patients, 15 (75%) patients received 3 cycles of therapy, 2 (10%) patients received 2 cycles, 1 (5%) patient each received 2 or 4 cycles of radiation, and 1 (5%)
