institutional experience to evaluate the impact of hypofractionation on pathological response and the acute RT related toxicity.

Material/Methods:

From August 2021 to July 2023, 14 patients with limb or trunk STS underwent hypofractionated preoperative RT (HFRT) in two treatment schemes: 30 Gy in 5 fractions (ultra hypofractionated RT, U-HFRT) and 42,75Gy in 15 fractions (moderate hypofractionated RT, M- HFRT). A favorable pathologic response (fPR) was considered as ≥ 80% of necrosis and/or percentage of viable tumor <25% on specimen examination.

Results:

All patients completed RT treatment with no interruptions. Seven patients (50%) were treated with U-HFRT. Median tumor size was 10,2 cm. Mean time between end of RT and surgery was 4.6 weeks. Nine patients (64,3 %) achieved a fPR. No patient developed ≥ grade 3 acute skin toxicity during RT or within 4 weeks after; 10 patients 71,4%) grade 1-2 acute skin toxicity. Three patients (21,4%) had acute major wound complications (including: 1 secondary intervention for wound repair and 2 cases of prolonged wound care).

Conclusion:

Preoperative hypofractionated RT treatment for STS is feasible, well tolerated, without greater acute toxicity compared with normofractionated RT, and associates encouraging rates of pathologic response.
