Material/Methods:

Between September 2017 and July 2023, we retrospectively analyzed clinical information for 32 patients (45 lesions) with SBRT of liver metastases in our center. After abdominal dampening with thermoplastic mask and 4DCT simulation with contrast or expiration CT with and without contrast and MRI fusion, patients received SBRT doses of 30-60 Gy in 3-5 fractions in Synergy®, Infinity® LINAC and Cyberknife®.

Results:

Median age was 70 years (33-90). The primary tumors were CRC in 43.75%. All patients had controlled or controllable primary tumor and 21 patients (65,6%) had received previous systemic treatment. Median follow-up after SBRT duration was 12,6 months (range, 1-57 months). After the follow-up period there were 53,12% distant relapses (17 patients). 14 patients had distant relapse at 1 month (2), 3 months (5) and 6 months (7). Median OS from initial diagnosis was 34 months (range 14,9-65,9 months). The univariate model indicated that the following were significant independent predictors of early distant relapse: female gender (p=0,01), colorectal primary (p=0,007), adenocarcinoma histology (p=0,039), low level of platelets before RT (p=0,018), and high level of INR before RT (p=0,005).

Conclusion:

SBRT is an effective treatment for patients with liver metastases. In particular, gender, primary tumor, histology, platelet count and INR are independent prognostic factors for early distant relapse. These data establish hypotheses for further studies.
