Radiotherapy for locally advanced pancreatic cancer: SBRT-like approach for all. Results.

Purpose/Objective:

Locally advanced pancreatic cancer (LAPC) currently lacks curable treatment options, and there is ongoing debate about the role of radiotherapy (RT) in this context. The tumor's location and size can limit the total deliverable dose, impacting both local control and the potential for adverse effects. By employing an SBRT-like immobilization and dosimetry approach, which includes simultaneous integrated boost and simultaneous integrated protection (SIB SIP) techniques, it is possible to escalate the dose while safeguarding nearby critical structures. This strategy can be applied in all clinical scenarios.

1- Describe the simulation phase, volume delimitation and dosimetric planning of the plans with SIB-SIP.

2- Analyze the results of local progression-free survival and overall survival (OS) in our cohort of 39 patients treated with normo (N-RT) (28 fractions), moderate- hypo (MH-RT) (10-13 fractions) and extreme-hypo (EH-RT) (5 fractions) fractionated radiotherapy.
