Conclusion:
We have successfully demonstrated the first clinical implementation of simulation-free Unity MR-Linac treatment for prostate SBRT, highlighting diagnostic PSMA-PET/CT scans can be used in-lieu of simulation-CT. The workflow on the MR-Linac was able to adapt to anatomical variations between the PSMA-PET/CT reference plan and fraction 1, including the insertion of rectal spacing after the PSMA-PET/CT. This workflow can benefit patients by eliminating simulation scans, reducing their overall amount of time spent at the clinic, and potentially expediting time to treatment. This workflow is now the standard of care for patients receiving prostate SBRT on the MR-Linac in our centre.
