ISUP, 14.1% of patients were found to have downgrading and 45.5% of patients had an upgrading regarding the biopsy.
At biopsy 100% of patients had clinical stage T2(a-c), after prostatectomy 51.9% had signs of extracapsularity (T3a) and 21.8% had seminal vesicle involvement (T3b). Seventeen patients out of 79 who had undergone lymphadenectomy had positive lymph nodes. No statistically significant differences were found between age and upgrading or upstaging as in other series however statistically significant differences were found between age and extracapsularity after prostatectomy in our series (p=0.041)
No differences were found between PSA and upgrading, but statistically significant differences were found between PSA at diagnosis and upstaging (p=0.003).
Conclusion:
although it may be due to selection bias since patients with biochemical recurrence candidates for radiotherapy were selected, up to 83.3 % of patients in our series had upgrading or upstaging. An accurate diagnosis is essential for the choice of the best treatment at each stage of the disease.
