An audit of seizure prevalence & anti-convulsant drug usage in newly diagnosed gliomas

Purpose/Objective:
Primary and metastatic brain tumours can present with seizures in about 35-70% of patients (1-3). The Society for Neuro-Oncology (SNO) and European Association of Neuro-Oncology (EANO) updated practice guidelines on anticonvulsant prophylaxis in patients with newly diagnosed brain tumours in 2021(4). Despite guidelines, the practice pattern for anticonvulsant drugs(ACD) remains heterogeneous with a wide clinical variation across centres. We audited our anticonvulsant drug practice in newly diagnosed gliomas to understand deviation from practice guidelines and its consequent economic burden. Additionally we investigated the relationship between the integrated molecular diagnosis and seizure incidence.
Material/Methods:
Retrospective chart review, after Institutional Review Board waiver, was performed on 1647 primary brain tumour patients seen between 2016 - 2022. Of these, 670 glioma patients evaluated for adjuvant radiation therapy and/or chemotherapy based on an integrated molecular diagnosis as per World Health Organization (WHO) Classification of Tumours of the Central Nervous System (CNS) 2016, who met our selection criteria of being >18 years and primary surgery as the first approach. Descriptive statistics were used to analyse the data. In addition to the 2021 SNO-EANO practice guideline adherence, cost of upfront and/or continued usage of anticonvulsant drugs was calculated for these patients.
