Purpose/Objective:

to describe the feasibility, toxicity, and disease outcomes in women aged over 65 with cervical cancer who underwent brachytherapy treatment.

Material/Methods:

We conducted a retrospective review of a single institution, analyzing women aged over 65 with locally advanced cervical cancer treated with radiotherapy or concurrent chemoradiotherapy (CRT) and brachytherapy (BT) between 2015 and 2021. We collected data on cancer, treatment characteristics, time to locoregional recurrence (LRR), distant metastasis (DM), and overall survival (OS) from the date of biopsy, Charleston comorbidity score (CCI), and toxicity. Descriptive statistics were calculated, and survival was analyzed using the Kaplan-Meier (KM) method.

Results:

Among the 110 identified women, the average age was 73 years [65-102]. Diagnostic delay, defined as the time between the onset of symptoms and cancer diagnosis, averaged 7 months [1-48 months]. Squamous cell carcinoma was the most predominant histological type, found in 94.5% of cases, followed by adenocarcinoma at 2.7%. Other histological types were less common, including small cell carcinoma (0.9%) and mesonephric carcinoma (0.9%). All patients underwent 3D external conformal radiotherapy and intracavitary BT using the Fletcher applicator. The median follow-up was 52 months for all patients (3-112). At the time of analysis, 13 patients (11.8%) had passed away, 95 (86.4%) were still alive, and 2 (1.8%) were lost to follow-up. The median OS, as calculated by KM, was 65 months. A complete response was achieved in 35 patients (31.8%). There were 9 local recurrences (8.2%), 3 lymph node recurrences (2.7%), and 9 metastatic relapses (8.2%). During the monitoring period, no grade III or higher toxicity was observed. Eleven patients presented with grade II rectitis, and 4 had grade II cystitis.

Conclusion:

our study highlights the efficacy and safety of brachytherapy in elderly women with cervical cancer, further Therapeutic trials dedicated to the elderly should be considered.
