Figure 2 provides a summary of clinical goals for the fractions delivered through the online workflow. Notably, with online full re-optimization, the results consistently match or surpass those of the initial plan, exemplified by the minimum dose delivered to PTV_5940 and the maximum dose to the brain.

Conclusion:

With this patient treatment we have demonstrated, for the first time, the clinical feasibility of delivering high precision DAPT treatments in the clinical environment. Our approach (full re-optimisation of plan before each fraction delivery) allows to at least maintain or improve the planned dose distribution by exploiting favourable patient anatomies on each delivery day. Based on this first clinical delivery of DAPT, we plan to extend its use to a larger patient population, to exploit the advantage of the increased conformality offered by DAPT also for deformable anatomies, which promises to provide even higher levels of precision and patient care.
