Nevertheless, visual outcomes are good, as even some eyes with centrally located tumours retained useful vision and 49% of all patients having no visual impairment (≥0.5) after five years.

HDR BRACHYTHERAPY OF THE EYELID: A TOOL FOR THE FUTURE

Purpose/Objective:

Eyelid malignances represent less than 3% of all head and neck skin cancers.

Mohs surgery or wide excision are the established primary treatments achieving local control rates of 95-98% in skin cancers. But surgery of the lower eyelid is not free of adverse effects especially on cosmesis (ectropion) and function (epiphora).

High dose rate (HDR) brachytherapy emerges as a possible alternative. There are few studies and the goal is to reach local control with the minimum cosmetic and functional alteration possible.

Material/Methods:

A total of 6 patients have been treated in our hospital from June 2019 to July 2023. with HDR brachytherapy with radical intention. Four men and two women, aged 46 to 91.

In 5 of the 6 patients the tumor was in the lower eyelid and 1 in the internal border of the eye. Three cases were operated, with positive margins and brachytherapy was postoperative. Two basal cell and four squamous cell carcinomas.

• Insert the folded abbocath with the guide into the lower eyelid, usually two parallel tubes. • Take off the needle and leave the guide. • Withdraw the button of the plastic tube and insert the two tubes into a small piece of dropper tube for fixation. • Place the buttons on the opposite side.

PLANNING CT: For the planning TC it is mandatory to delimit the tumoral area with an opaque material. The separation of the slides should be 1-1.5 mm due to the horizontal arrangement of the tubes.
