The study population included ten patients, with 8 males and 2 females, and an average age of 69 (Table 1). Five patients were diagnosed with BCC, and five with cSCC. Prior surgical intervention was noted in 3/10 patients. Lesions measuring less than 3cm were found in 5/10 patients. Varied dosages were administered: 40 Gy in 10 fractions (3 patients), 42.5 Gy in 9 fractions (1 patient), 40.5 Gy in 9 fractions (1 patient), and 45 Gy in 9 fractions (5 patients). The median Clinical Target Volume-Therapeutic (CTV-T) volume was 10.69cc [3.75cc-63.16cc]. Median Dose covering 90% of the target volume (D90) was 40.77Gy [35.59-47.81]. Volume receiving 150% of the prescribed dose to volume receiving 100% of the prescribed dose (V150/V100) median ratio stood at 0.68 [0.63-0.79]. One month post-treatment, all 10 patients demonstrated partial response. Complete response was observed in 6/10 patients three months post-treatment. One patient experienced a local relapse six months after treatment, necessitating partial rhinectomy. At a median follow-up of 12 months, 4/10 patients remain in complete remission. Acute toxicity primarily consisted of grade 3 dermatitis in 6/10 patients. Aesthetic outcomes were noteworthy: 9/10 patients achieved GAIS grade 1, and 1 patient achieved grade 2.
Conclusion:
HDR iBT emerges as a viable alternative in treating early-stage nasal BCC and cSCC, with a focus on precision and aesthetic preservation. The study's strength lies in the homogeneous application of this rarely utilized technique, reducing long-term septal risks through non-piercing implantation. Although local control and aesthetic outcomes are promising, further research with extended follow-up is necessary for a more robust understanding of long term implications.
