Conclusion:

We have demonstrated the possibility to perform palliative oART treatment of bone metastases in the spinal vertebrae using a standard C-arm linac and our normal treatment planning system, without the acquisition of a pCT. Total time consumption for the whole procedure was acceptable. With the introduction of the RAPID-care phase II trial, we will further explore the potential of oART in palliative RT for bone metastases and work towards a reduction in the total time that the patient has to stay in the hospital.

Purpose/Objective:

In 2020, the Children’s Oncology Group (COG) conducted a survey of 152 COG institutions on the practice patterns in pediatric total body irradiation (TBI) (ref 1). Almost all the radiation oncologists-respondents (98%) desired further refinement of conventional TBI techniques, most of them (79%) supported the investigation of new techniques to lower lung doses, and 83% supported the investigation of VMAT or helical Tomotherapy. But, as also shown by the survey, the supply does not meet the demand: only a handful of COG institutions have implemented VMAT TBI. We created the autoplanning scripts and shared them with the public to make the VMAT TBI more wide-spread. Our VMAT-TBI technique together with the scripts was recently included on the COG ASCT2031 trial. Here, we report on our 4-year experience of treating VMAT TBI using the scripts at our institution.
