Figure 2. Exemplary images of patient # 9 dose distribution of the standard treatment reference plan based on a dedicated planning (A), standard adapted MR-Linac plan (B), simulation-free reference plan based on dCT (C), simulation-free adapted plan (D) and DVHs of both reference plans (E) and both adapted plans (F). Solid lines indicate the standard treatment plans and dashed lines the simulation-free plans. Differences in the standard and simulation-free reference plans due to different organ filling levels or patient positioning could be compensated for in the online adaptive MR-guided workflow.

Conclusion:

In this study, we investigated the feasibility of simulation-free planning for online adaptive MR-guided RT in prostate cancer. The feasibility of a simulation-free approach using dCTs for planning is substantiated by comparable treatment plan quality observed for both the reference and adapted RT plans in a curative setting for PC. Despite very promising first results in this retrospective in silico study, these results warrant further validation in a prospective clinical setting.
