EQD2-based re-irradiation treatment planning, accounting for previously delivered dose in a novel pathway, is feasible for lung cancer re-irradiation. The novel optimisation pathway takes into account the full 3D information on the previous dose distribution, and this may generate better-optimised re-irradiation plans, delivering more dose to the target by safely increasing dose to OAR whilst remaining below toxicity concern. Consequently, clinically acceptable plans were more likely to be achieved compared to planning using a standard manual approach, and EQD2-based plans were preferred in blinded evaluation. The robustness analysis demonstrated that EQD2-based plans were generally equally robust, and therefore not ‘over-optimised’ compared with the standard manual plans.
