Evaluation ability of dual-Energy CT for the extent of osteolytic spinal metastases infiltration

Purpose/Objective:

The international consensus guidelines for target volume definition in spinal stereotactic radiosurgery (SRS) recommend that the gross tumor volume (GTV) be established using all available imaging modalities, including MRI and CT [1]. For the clinical target volume (CTV), the vertebra is divided into six segments: vertebral body, spinous process, bilateral pedicles, and bilateral transverse process/lamina; the segment containing the GTV and the adjacent segments are recommended to be included in the CTV. Therefore, the accurate evaluation of segments, including the GTV, using MRI is necessary. However, in actual clinical practice, MRI is sometimes not possible due to several reasons such as pain caused by bone metastasis or financial constraints. Dual-energy CT (DECT) is an imaging technique that can estimate material density and effective elemental composition from the difference in absorption values between CT images taken at two different voltages.
