Interim PETCT IGOART Pelvic Malignancies: A Guide to Precise Futuristic Dose Escalation Strategies

Purpose/Objective:

Concurrent chemoradiation followed by Brachytherapy remains the standard of care for locally advanced carcinoma cervix.

FDG PET CT Scan is a widely used functional imaging modality depicting the metabolic activity of the tumour for staging, treatment planning and response assessment of various malignancies.

However, the precise treatment delivery is still not achieved in many cases because of the motion of the pelvic structures. To overcome this, multiple techniques like bladder and bowel protocols, IGRT, Adaptive radiotherapy have evolved.

The importance of IGRT in pelvic malignancy, especially in cervical cancer is well established with Image guided Brachytherapy being the standard of care in cervical cancers.

Online adaptive radiotherapy is an integrated suite of tools for adaptive workflow from initial planning to on couch adaptation and treatment.

Here, we attempted to assess the correlation between the clinical and the metabolic response in patients of Carcinoma cervix being treated with online adaptive radiotherapy with SIB technique using interim PET CT scans.

Material/Methods:

We treated 5 patients with a median age of 50 years (38 – 56 years) with locally advanced cervical cancer with IGOART with PET CT and MRI based planning with a customised bladder protocol. All the patients underwent EBRT with IGOART and concurrent chemotherapy with weekly Carboplatin followed by HDR ICBT.
