We show that high-risk PCa patients classified according to the 3-tier NCCN system benefit from further stratification using the CAPRA score or the 5- tier NCCN stratification method. Patients with a CAPRA score ≥6 and those classified as very-high risk by the NCCN had a higher risk of BCR, metastasis, and death. This subset of patients might benefit from further intensifying their investigations and treatment, underscoring the importance of ongoing research. Given the comparable performances of both predictive tools, the choice between them may rely on personal or institutional preferences.
