Conclusion:
Although the discrimination ability of the DN40 model was slightly lower at external validation, this was statistically comparable to that of a DVH-based random forest model for the same dataset. This indicates that, in addition to potential model overfitting issues, the small drift in AUROC was also likely driven by the dosimetric characteristics of the external dataset. The predicted probabilities at external validation were not well calibrated even after Platt scaling. This could be due to data distribution differences between the two datasets. Future work will aim to improve the generalisability of the model by re-training it on the combined dataset and repeating the external validation exercise on a third independent cohort. Data from the PREDMORN study [3] will provide such an opportunity.
