Conclusion:
Neglecting the variability of proton RBE can result in suboptimal treatment plans, underestimating the detrimental impact on normal tissue. Optimization of proton LET d or track ends can mitigate the dose burden induced by variable RBE and associated NTCPs, while maintaining clinical acceptance. The achievable NTCP reduction is influenced by several factors; for instance, a greater reduction was observed with a 3- compared to a 2-field plan. Further treatment planning studies as well as analyses of clinical toxicity data are necessary to fully exploit the potential and overcome the challenges of these promising strategies for optimizing proton RBE in the clinics.
