Conclusion:
While in current literature, both HPV negative status and higher extracellular diffusion in terms of ADC are correlated to poorer response, in our cohort of HPV negative patients, we found that only a lower extracellular diffusion in terms of D was related to poorer response. Moreover, the correlation was in the opposite direction as expected based on previous literature that did not correct for HPV status. These findings suggest that ADC is indeed a surrogate for HPV status instead of an independent response predictor, and highlights the need of applying NG-IVIM DWI instead of conventional ADC DWI for response prediction.
