For many failure modes, the risk increases in the data transfer and plan preparation phase of measurement-based PSQA, while this occurs for only a few failure modes in log file-based PSQA. Both PSQA methods have a similar overall impact on risk when the entire PSQA process is considered. Most failure modes are decreased by PSQA, while fewer are impacted by the secondary dose calculation. However, there is a group of failure modes that are not substantially impacted by either measurement-based or log file-based PSQA, but they are impacted significantly by the secondary dose calculation. The presence of these failure modes suggests that a combination of PSQA and secondary dose calculation provides greater risk reduction than any one mitigation strategy alone.
