Conclusion:

We could not validate a three-level Geometric Score due to low-size on the LR class, while we could confirm in and independent population that a large GTV size coupled with an incomplete (<70%) covering of GTV from the SIB highly increases the risk of LF. Complete coverage of GTV from the SIB would be of clinical relevance for patients with large macroscopic tumors. The Geometric Score could be used to select patients effectively benefitting from online tumor tracking, allowing a reduction of the SIP and a consequent decrease of GTV left uncovered by the SIB.
