Immobilization of head-and-neck cancer patients using a Double Shell Positioning System

Purpose/Objective:
New fixation material, a Double Shell Positioning System (DSPS) for head-and-neck cancer patients was introduced in our department in May 2023. This system can be combined with a conventional head coil in the MRI for optimal image quality and workflow in contrast to the conventional positioning, Posicast 5-point masks with individual head-support by MacroMedics BV (Moordrecht, The Netherlands). The aim of this study was to compare the setup variation and intrafraction motion between a group of patients with a closed DSPS mask with the eyes open and a group with an open-face DSPS mask. In addition, this data is compared with the Posicast from the study by Houweling et al. in 2010.
Material/Methods:
Fifteen patients with head and neck cancer, treated with curative intent were included in the pilot. Patients who also received chemotherapy during radiotherapy were not included in this pilot.
Group with closed DSPS masks:
67 fractions

• 8 patients, of which 5 larynx carcinoma and 3 oropharynx carcinoma

39 fractions
