RTT - Education, training, advanced practice and role developments
ESTRO 2024
From January to March 2023, 200 residents and young doctors registered for two delineation cases. Regarding the anal cancer case, similarity for the GTV_T, CTV_HR and GTV_N were relatively low with the mean DICE scores reaching 0.44 (CI95% 0.399-0.48), 0.53 (CI95% 0.48-0.57) and 0.31 (CI95% 0.24-0.37). Based on the analysis of the Ref-Stack and Stack-Ref maps, the readers underestimated the GTV_T especially at the upper and lower parts as well as circumferentially while overestimating for 50% of them the CTV_HR at the middle level. For the lung cancer case, DICE scores ranged from 0.42 (CI95% 0.36-0.48) for the CTV_N to 0.72 (CI95% 0.68-0.75) for the CTV_T. The CTV_N Ref-Stack map revealed that less than 10% of the participants correctly defined the anterior limit of the right pretracheal lymph node area. DICE scores for the H&N case were relatively high except for CTV_N_Left reaching a DICE of 0.71 (CI95% 0.66-0.77). The CTV_N_Left Stack-Ref map revealed that approximately 60-70% of the participants included the left Ib area whereas it was not included by the expert. Finally, for the prostate bed case, the DICE for the prostate bed delineation reached 0.64 (CI95% 0.60-0.67), the Ref Stack map revealing that less than 20% of the participants achieved a correct delineation of the antero-lower prostate bed, especially behind the pubic symphysis. Concerning satisfaction survey, 98,1% wished to renew the delineation workshop concept for another course and 83% were satisfied or very satisfied by the online platform.
Conclusion:
Delineation workshops are a great pedagogical tool for spreading and maintaining knowledge with positive feedbacks of residents. Similarity metrics provide a general overview of the overlap but provide no information on the spatial distribution of mis-segmentation. When compared to the expert delineation, probability maps should help to customize webinars and workshops to participants’ needs.
