Material/Methods:
At the time of implementation, the department housed 2 Varian TrueBeam linacs, treating approximately 105 patients daily. In 2018 one TrueBeam was commissioned by the local physics team for stereotactic radiotherapy for brain and bone sites. All protocols and work instructions were adopted from state-based documentation for the relevant technology, including simulation, contouring, image-guidance and treatment. A multi-disciplinary team approach was taken, with support from the Victorian GenesisCare network. Local staff including radiation oncologists and radiation therapists underwent training in prescribing, contouring, simulation, and treatment. Experienced metropolitan radiation therapists provided on-site support during the implementation phase. Local radiation therapists were selected to be subject matter experts and provided with further training. Stereotactic treatment planning is centralised within GenesisCare Victoria, performed by a dedicated team of highly experienced radiation therapy planners. All stereotactic plans are created and checked by the stereotactic planning team, who also provided initial onsite support around patient positioning, patient selection, image fusion and contouring. Further upgrades were implemented over the 5 years including 4DCT scanning for lung, changeover to the Varian Eclipse planning system, and installation of a Brainlab Exactrac imaging system, which allowed for progression to single fraction SRS for suitable patients.
