Impact of clinical parallel workflow on online adaptive MRgRT: a treatment Time Analysis

Purpose/Objective:
Magnetic Resonance guided RadioTherapy (MRgRT) allows high-quality MR imaging at any time during treatment, enabling daily online adaptive RT (oART). oART is based on the re-contouring of treatment volumes and rapid re optimization of dose distribution to account for daily changes in the patient's anatomy.
Different studies show that oART leads to significant dosimetric advantages, both in terms of target coverage and organs at risk (OARs) dose sparing.
However, the additional steps required for oART can significantly prolong the total treatment time, posing a challenge for daily clinical implementation and reducing patients’ compliance to this innovative approach. Technological developments in MRgRT are focusing on improving the system's capabilities and reducing the overall treatment time introducing new procedures such as the clinical parallel workflow (CPW). CPW enables radiation oncologists (RO), physicists (MP), and radiation therapists (RTT) to divide their tasks and work simultaneously (Figure 1). This feature facilitates the parallel execution of all the oART steps that would typically be performed sequentially.
This study aimed to evaluate the impact of the CPW on the overall treatment time by analyzing each step of the MRgRT workflow on a 0.35 T MR linac.
