The current retrospective study highlights the importance of radiation dose to OARs (heart, lungs, and immune cells) for oncologic outcomes like BCSS, OS, risk of LRR and DM in BC patients treated with adjuvant RT. Considerably, future studies reducing dose to OARs with advanced radiation techniques (like proton therapy) could provide more definitive evidence whether the irradiated OARs themself are risk factors for worse oncologic outcomes. In conclusion, the current study suggests that to further improve BC survivorship, mean dose to OARs needs to be minimized.

Purpose/Objective:
Breast carcinoma is the most common cancer in women worldwide. Rates of mastectomy with immediate reconstruction (IR), based on implant or autologous tissue as surgical treatment for patients with early breast cancer are increasing [1,2]. Indications for postmastectomy radiotherapy (PMRT) are based on tumour-related features and other prognostic factors. Evidence suggest that PMRT reduces the risk of local recurrence and mortality in indicated cases [3-5]. Although some studies suggest that PMRT in context of reconstruction, regardless of type and timing, increases the risk of complications, others claim that autologous implantation has fewer complications and a better long-term cosmetic outcome compared to expansor or silicone prosthesis implants [6 9]. The aim of our study is to analyse local and regional control rate, distant recurrence, complications and adverse effects in breast cancer patients undergoing mastectomy with IR who undergo treatment with external radiotherapy (ERT) at our institution.
