after TNT compared to CRT [25% vs. 2%; P interaction =0.01] ( Table 1 ). Furthermore, a distal margin of ≤10mm was reported for 36% (73/203) of Dutch SPS patients, compared to only 10% (31/301) in the other countries (p<0.001). Other evaluated characteristics were similar between treatment groups with respect to LRR.

Conclusion: The difference in LRR between TNT and CRT mainly occurred in patients treated with sphincter-preserving surgery. Our results indicate that after SCRT followed by preoperative chemotherapy, a distal margin of ≤10mm is insufficient to achieve low LRR rates.
