Speaker Abstracts Motion management strategies when treating thoracic oligometastases with SBRT: An RTT perspective Ina Nilo Radiation Oncology, University Hospital Zurich, Zurich, Switzerland

Abstract:

In high-dose radiation therapy, effective motion management is crucial for optimizing the targeting of the treatment volume while minimizing the risks of radiation exposure to surrounding healthy tissues. The use of Magnetic Resonance (MR) Linear Accelerators has revolutionized the way motion is addressed during radiation therapy treatments.

This presentation explores motion management techniques specifically tailored for MR-linac systems.

We will discuss the integration of real-time imaging capabilities afforded by MR-linac technology. We will cover simple cases involving gating one volume with one plan at a time, as well as multiple volumes with one plan, both for adaptive and non-adaptive radiation treatment. In addition, we will address some troubleshooting issues and the most common challenges. Regarding treatment gating, we will highlight its impact on patient safety and treatment efficacy. Moreover, we will present case studies demonstrating the application of these motion management strategies in various clinical scenarios, including lung, liver, and pancreatic cancers. Through comparative analyses, we will illustrate how MR-guided motion management leads to enhanced accuracy in dose delivery.

Additionally, we will discuss how we decide on the appropriate gating strategies during simulation, so we are better prepared when patients come for their first radiation treatment.

Finally, we will address future directions of motion management in radiation therapy with MRI, discussing the challenges that remain and the potential for further technological advancements. This presentation aims to provide insights into the transformative role of MR-linac and MRI systems in enhancing precision medicine and setting a new standard for patient-centered care in radiation therapy
