Conclusion: Personalized high-dose SBRT with extended fractionation interval is an effective and well-tolerated treatment for extracranial tumors, providing high rates of local control and symptom relief with limited toxicity. Due the long overall treatment time, daily online adaptive planning is essential for managing interfractional changes in tumor volume. This regimen appears particularly beneficial for elderly patients, large lesions and tumors located near sensitive and/or pre-irradiated organs. While weekly SBRT shows promise in managing patient tolerance and minimizing side effects, ongoing clinical trials are essential to fully establish its role and optimize fractionation schedules based on tumor-specific responses.
