Conclusion: Despite initial management as recommended, one-third of our patients still relapsed. Relapses are local, out of field and within the first year post RT, but can be effectively salvaged with RT or surgery. Relapse awaiting RT is not uncommon and seems to reflect a more aggressive disease, leading to ganglionar or metastatic spread, reflecting the need for close and periodic local examination after surgery and shortening time to RT. Relapse leads to patient's death, suggesting a need for treatment intensification, although local salvage with RT seems an optimal management for elderly patients due to the low toxicity profile.
