Conclusion: Routinely performed radiotherapy planning CT scans can be leveraged to obtain a quantitative estimate of patient fitness, namely SMI, which is associated with early mortality following definitive chemoradiotherapy for oesophageal cancer. This study highlights a potential method of patient stratification to aid selection of radical treatments. This may reduce delivery of futile treatments, reduce toxicities and improve outcomes by further personalising patient care. Validation of these findings on larger prospective cohorts of patients over the age of 70 is required.
