Conclusion: This large multicenter cohort study demonstrates that consolidation (C-)RT following induction chemotherapy for LAPC is feasible and well-tolerated, offering encouraging survival outcomes. The significant survival benefit observed in patients achieving surgical conversion suggests that (C-)RT may help identify candidates for curative-intent surgery. The comparable outcomes between chemotherapy regimens provide flexibility in treatment selection based on individual patient factors. However, the lack of reliable predictive biomarkers highlights the need for better patient selection tools. These findings contribute valuable real-world evidence to the ongoing debate regarding optimal sequencing of multimodality therapy in LAPC.
