Conclusion: Oncological outcomes for resected patients with adverse prognostic factors treated with adjuvant RT are similar to those of unresectable patients treated with chemoradiotherapy. If achieving negative margins and pN0 is unlikely, consideration should be given to excluding surgery for these patients, potentially prioritizing alternative treatment strategies such as chemoradiotherapy.
