Conclusion: Around a quarter of patients with positive PSMA scans had evidence of local uptake at the time of first failure after sRT, suggesting methods to improve local control may have utility. Although Gleason score was predictive for LF, overall, we were unable to find factors at the time of sRT that strongly predict for LF risk. This may in part reflect non-specificity of PMSA uptake, although in the minority of patients with confirmatory MRI/biopsy, PPV was reasonable.
