Conclusion: The 2% tolerance level for beam output checks with the MVI was confirmed in long-term clinical practice. Incidentally, we discovered that cross calibrations should only be approved if their accompanying output deviation differences are within 0.6 pp. Our procedures were updated accordingly.

This system leverages comprehensive data collection and real-time analytics to identify trends, prioritize risks, and inform proactive interventions. By harnessing detailed incident data, we aim to enhance clinical safety, optimize treatment workflows, and support evidence-based decision-making in patient care. Material/Methods: We developed a web-based system enabling multidisciplinary staff to report incidents in real-time, supporting anonymous submissions with detailed data capture.
