81 patients required myeloablative conditioning regime, most 12Gy in 6 fractions, n=78, lung sparing to to reduce lung dose under 8Gy. Remaining 16% received reduced – intensity conditioning (2-4Gy). Most patients underwent allogeneic SCT (n=80), 5 haploidentical and one autologous. 26 patients relapsed in a median time of 10 months, 20 of them passed away. Other causes of death were early complications due to the transplant (n=14), neutropenic fever and mediastinitis. Acute and chronic graft versus host disease (GVHD) are described in Table 1 . Among living patients, median follow – up was 6.9 years with and OS of 56% and 50% at 2 and 7 years, respectively (Figure1) . On long – term follow – up no patients reported pneumonitis or nephritis, one required cataract surgery.

Conclusion: SCT, often preceded by TBI-based conditioning regimes, has significant survivorship challenges. The present 20 – year experience using conventional 2D TBI has been a safe technique with relatively low rates of radiation – related toxicities.
