Purpose/Objective: We report the outcome of prostate cancer patients, treated with Helical Intensity Modulated Radiation Therapy, using simultaneous integrated boost SIB- HIMRT, regarding toxicity and biochemical control.

Material/Methods: A total of 68 patients with localized and locally advanced prostate were enrolled in a prospective study. The prescribed total dose was 67.5 Gy, which was administered in 25 fractions of 2.7 Gy each. Androgen deprivation therapy ADT was prescribed for 6 months for patients with unfavorable intermediated risk and for 24 months for all patients with high risk. SIB- HIMRT was used for all patients, and the image guided radiotherapy “IGRT” was performed using megavoltage CT “MVCT”. Toxicity was measured using the common terminology criteria for adverse events in V.4.03 ” CTCAE”. The Phoenix definition of PSA nadir + 2 ng/ml was used to define biochemical failure. Results: The study found that the rate of acute genitourinary GU toxicity grade 2 or higher was 13% , while the rate of gastrointestinal GI grade 2 or higher was 7%.No patient experienced grade 3 acute toxicity. Late GU and GI toxicities of grade 2 or higher were observed in 12 % and 4% of patients respectively. 4.4% patients experienced grade 3 late GI. There were no cases of local, regional or distant failures and all patients were still alive at the last follow up. Conclusion: In our study, we observed positive outcome of toxicity for all of our patients who were treated with SIB HIMRT, regarding the oncological results it is early to deduce the effectiveness.
