Results: The initial catalogue comprised 213 PSIs in four main categories (human resources, risk and quality management, institutional culture and patient-specific processes). Of these, 187 were retained after round 1, with or without changes, including 7 newly identified PSIs. After round 2, 158 PSIs were included in the final list. In round 1, 84% of PSIs received at least 6/9 points in the rating for relevance, increasing to 94% in round 2. The mean feasibility and ease of data collection was 2.9 and 3.1 out of 4. Table 1 presents exemplary PSIs and their ratings throughout the Delphi process.

Conclusion: This study presents the first comprehensive consensus on PSIs in radiation oncology, incorporating expert feedback from all professional groups. Using the Delphi method, we made sure that all PSI are relevant in radiation oncology
