Conclusion: This study systematically examined the differences in dose distributions between HNC patients treated with definitive photon and proton therapies. Using the biological dose in part mitigates OAR dose reduction between these two modalities, though significant dose reductions remain in OARs for protons. These OAR reductions may explain the reduced toxicity seen on randomized trial. However, increased biological dose, especially to the brachial plexus and mandible, should be considered.
