Conclusion: Max_heart_depth is a simple and effective predictor of MHD, strongly influenced by chest-wall and breast anatomy below the IMN PTV. While automated scripting was used in this study, in the clinical setting, max_heart_depth can be manually measured on planning CT scans using the ruler function, requiring only the posterior border of a tangential field. The model predicts MHD within +/-1.6 Gy of a wide tangential plan whereby no compromise to target volume coverage is made, enabling timely, resource-efficient decisions about heart-sparing techniques in left sided breast cancer care.
