Conclusion: Performance characterisation of the first open-source couch tracking system demonstrated lower geometric targeting error. Prospective clinical use of the couch tracking system for real-time adaptive radiotherapy is approved within a Real-time IGRT Master Protocol clinical trial. The couch tracking system hardware design and software are available from https://github.com/Image-X-Institute/couch-tracking.
