Conclusion: This is the first time, to our knowledge, that an AI-based analysis of contouring consistency detected a clear impact on toxicity. The clinical meaning of this finding is unclear as it may depend both on inter-observer contouring variability and/or in the individual attitude of clinicians in adapting their contour aiming to reduce toxicity in specific patients through a less generous CTV contouring. Further investigation is ongoing to consider the risk of local relapses, aiming to explore quantitative methodologies to support the definition of optimal contouring policies.
