Material/Methods: Between 2013-2022, 120 patients were treated with HDR salvage prostate brachytherapy following recurrence after EBRT or LDR/HDR brachytherapy. Multiparametric MRI, PET (PSMA or Choline) and whole-body MRI defined locoregional status and excluded metastases. All patients underwent template biopsy (20 Barzell zones). Table 1. shows the NCCN risk group at first diagnosis.

14 patients (11.5%) had seminal vesicle only relapse. 78.8% received 19Gy/1# and 17.8% received 26Gy/2#.
