Purpose/Objective: To describe the results and improvements after applying the tools of the Risk Matrices in Radiotherapy Advanced Techniques (MARRTA) project to analyze and calculate the risk associated with initiating events in the patient care process, as well as the SAFRON system to notify risks.

Material/Methods:

A MARR safety analysis system has been used since 2018. It was upgraded to the MARRTA system in 2022, which is adapted to advanced techniques. An interdisciplinary group of physicians, physicists and technicians meets weekly to perform a proactive analysis, calculating risks based on frequency, impact and probability of barrier failure in state-of-the-art accelerators (MARRTA), and also a retrospective analysis, using an anonymous incident reporting system (SAFRON), following 2013/59/EURATOM directives, basic security directions to protect against ionizing radiations. The system consists on describing the proccess to analyze, identifying consecuences of such procces, determinating the likely mistakes associated to the latter (initiating events) and determining the existing barriers and frecuency reducers.

Results:

After analyzing the yearly results, the interdisciplinary group proposed actions such as improvement of the work environment and spaces, implementation of new technologies and improvement of protocols or cross-checks were implanted.
