This study aimed to evaluate clinical outcomes, survival, and toxicity profiles in endometrial cancer patients aged over 80 years treated with postoperative radiotherapy across multiple Spanish hospitals. Material/Methods: This retrospective study included 353 patients aged ≥80 years diagnosed with endometrial cancer and surgically treated between 2003 and 2019 across 19 Spanish hospitals. Adjuvant radiotherapy (external beam radiotherapy [EBRT] and/or brachytherapy) was delivered based on clinical risk factors. Outcomes assessed included local control, disease-free survival (DFS), cancer-specific survival (CSS), and overall survival (OS) at 1, 2, and 5 years. Chronic gastrointestinal and genitourinary toxicity was recorded. Kaplan-Meier survival analyses and Cox proportional hazards models identified prognostic factors.
